DESCRIPTION OF THE ADMINISTRATIVE SERVICE FOR THE ISSUING DOCUMENTS CONFIRMING SPECIFIC LEGAL FACTS

1. | Administrative Service Title | Issue of documents confirming certain legal facts. |
2. | Administrative Service Description | Requests for issuing documents confirming legal facts are accepted only in writing. Requests can be submitted in person at the Klaipėda Regional State Archive (hereinafter - the Archive) or the Archive's Tauragė or Telšiai branches (hereinafter - the Branch), by mail, through a courier, via the Electronic Archive Information System or through the Electronic Government Gateway. Requests submitted by e-mail must be signed with an electronic signature. Responses to requests are provided in the same way in which the request was submitted, unless the service recipient requests to receive the response in another way. In the Archive or Branch, individuals are served according to the “one-stop” principle. Individuals are served and provided with information by the Archive or Branch receptionist, who performs the “one-stop” customer service functions. |
3. | Information and documents to be submitted by a person | Application (the application must be signed, indicating the person's name(s), surname(s), place of residence (if a natural person applies) or name, code, registered office address (if a legal person applies). A document confirming the identity of the person or a copy thereof, if the application is sent by post. Documents confirming the representation and the identity of the representing person or copies thereof, if the application is sent by post (when the information about the person is applied for by his representative). |
4. | Information and documents that the institution (the official examining the application) must receive | Application, a copy of the document confirming the identity of the person, copies of the documents confirming the representation and the identity of the representing person (when the information about the person is applied for by his representative) and, taking into account the content of the application, the necessary information and (or) documents or copies thereof. |

6. | Duration of Administrative Service Provision | The administrative service is provided within 20 working days from the start of the administrative procedure. If, due to objective reasons, the administrative service cannot be provided within this period, the public administration entity that initiated the administrative procedure may extend the deadline for providing the service, but not longer than for an additional 10 working days. |
7. | Cost of providing Administrative Service (if the service is provided for a fee) | Document search is charged in accordance with the list of prices for paid services of the State Archives, approved by the Order of the Chief Archivist of Lithuania of July 8, 2022 No. VE-30 “On approval of the list of prices for paid services of the State Archives and the description of the procedure for providing paid services of the State Archives”. If the application is for documents confirming the length of service and (or) the salary received in the Archives or the Branch, the administrative service is provided free of charge. |
